fix: improve rename popup

Summary

• replace the cloud rename modal with a compact popup width
• restore an explicit rename action button instead of save-on-blur
• keep the rename popup titled for document and folder rename flows

Testing

• npm run lint -- src/cloud/lib/hooks/useCloudResourceModals.tsx
• ./node_modules/.bin/tsc --noEmit --pretty false
• git diff --check
